Any pension information about a member is confidential under the Pension Law. In order for the Pension Fund to share information regarding a specific member with someone other than the member, the member will need to execute a Records Release Authorization Form. A link to the form is below. Once the document has been signed by the Member and notarized, please submit the form to the Pension Fund Office.
